Medical RecordsYOUR CONFIDENTIALITY is one of our priorities at the Health Center. All of your medical visits and records are held with the strictest confidentiality. No one but the student has access to their records until a medical record release is completed. If you need a copy of your personal medical records, please complete an Authorization to Release Medical Information from the University Health Center form in its entirety and turn it in at the front desk of the health center. The first copy of your medical record is free. Additional copies are $18.00. You may also fax or mail the form. |

Getting your previous health records to the University Health Center:If you need records from an outside practitioner, please fill out an Authorization to Release Medical Records to the UO Health Center in entirety so that our staff may obtain your previous medical records. Medical Records FormsAuthorization for Release of Confidential Medical Information from the University Health Center Our assistance is available M,W,H,T,F 8:00 to 5:30 and T 9:00 – 5:30 with these forms. Please allow ten business days for the processing of your request. A confidential medical record is established and maintained for every patient who receives care at the UHC. These forms are in Adobe PDF format. You will need the free PDF viewer to use these forms.
How do I obtain an Itemized Insurance Statement?STUDENTS WITH PRIVATE HEALTH INSURANCE may request an itemized statement to submit for reimbursement of charges from the University Health Center. All health insurance plans are different. It is best to call your insurance company and let them know health care is available on campus. They will be able to tell you what your plan will cover. You may request a statement in person with your photo I.D. at the front desk. Requests can also be made by mail or fax, signed and dated by you, along with your printed name, student ID number and complete mailing address. Due to privacy laws, the Health Center cannot respond to phone requests or requests from parents. Only YOU, the student, can access your medical information.
