Your confidentiality is a priority at the University Health Center (UHC). We are committed to upholding all legal and professional obligations to protect the confidentiality of your health records. Confidentiality is central to the effective provisions of health care services. A confidential medical record is established and maintained for every patient who receives care at the University Health Center. To obtain a copy of your medical records:
Please complete "Release of Information" form (link for form below or log into the Patient Portal and it is located under the forms tab). Once you have completed this form in its entirety, submit it through the Patient Portal or bring a paper copy to the front desk of the Health Center.

Assistance with these forms is available Monday through Friday from 9:00 to 5:00 p.m. Please allow 10 (ten) business days for the processing of your request.
