Family Planning Expansion Project - FPEP Guide

The University Health Center is pleased to participate in the Family Planning Expansion Project--this great federal program provides FREE family planning services, birth control and reproductive health care to men and women. Our on-line FPEP Guide, located below, should answer most of your general questions regarding the program. For specific questions please feel free to call or e-mail: pgregg@uoregon.edu .

Enrollment Form Pages 1,2, & 3 are required. You may fill out pages one or two in English or in Spanish. Page three is available only in English:

Note: If you were born in Oregon and would like University Health Center staff to try to verify your citizenship electronically, please print and fill out the Oregon Birth Information Form. This form will be used to search for your birth records through Oregon Vital Records. If an electronic match is made, you will not need to provide your original birth certificate. Bring the completed form with the FPEP Enrollment forms to the University Health Center Main Reception Desk.

What is FPEP?

FPEP is the acronym for the Family Planning Expansion Project that was implemented in January of 1999 as a five-year Medicaid project to expand coverage for family planning services. The goal set forth for FPEP is to reduce unintended pregnancies and improve the well being of children and families.
