INSURANCE CARDS 

Domestic & Law students - If you enroll by mail, your insurance ID card will be mailed to you. If you enroll at the Health Insurance Office, you will be given an insurance ID card at the time of enrollment.

International Students - You will be billed for health insurance on your tuition bill. Your insurance ID card will be mailed to you once you have paid your health insurance premium.

All Students

1. Your assigned university ID (your ‘950/951' number) will be your insurance ID number.

2. Write your name and your university ID number on your insurance card. The insurance company will not be able to identify you by your Social Security number.

If you lose your insurance ID card, please contact the Health Insurance Office on campus to obtain a new card.

Make sure you update your DuckWeb information on-line to include a local mailing address so that your insurance ID card can be mailed to you.
