Pre-Existing Conditions

A pre-existing condition is any injury, sickness or condition that was diagnosed or treated within 6 months prior to your effective coverage date on this insurance plan. Treatment includes prescriptions for treatment of injury, sickness or a condition.

Coverage for pre-existing conditions will be excluded for a period of six months following the effective date of your coverage under the UO plan.

Waiving Exclusion

This exclusion can be waived if you submit a “Certificate of Creditable Coverage” from your prior insurance company showing:

  1. At least 6 months continuous coverage.
  2. No more than a 63 day break in coverage between expiration of your previous health insurance coverage and your enrollment on the UO health insurance plan.

What Is a Certificate of Creditable Coverage?

A “Certificate of Creditable Coverage” is a document of proof of prior health insurance coverage that was mandated by the federal Health Insurance Portability and Accountability Act of 1996.

Obtaining a Certificate of Creditable Coverage

Students do not need to have their “Certificate of Creditable Coverage” at the time of enrollment. The purpose of requesting a “Certificate” is to show the beginning and ending date of your prior coverage. Wait until after the previous coverage has ended before requesting the “Certificate.”

Some insurance companies will automatically mail a Certificate of Creditable Coverage once the coverage has ended. If not, a “Certificate must be requested from the insurance company. Once the student has received the “Certificate”, it should be submitted to the Health Insurance Office on campus.
